Why it happens
Almost always a provider/billing delay - the claim wasn't sent within the payer's filing window.
What to do
This is usually the provider's responsibility, and they often cannot bill you for a timely-filing denial. Ask the billing office to appeal with proof of timely submission; you should not pay this.
