Frequently asked questions
Quick answers about privacy, accuracy, cost, and what Denial Decoder can - and can't - do for you.
Is this legal or medical advice?
No. Denial Decoder is an educational tool that helps you understand your denial letter and draft your own appeal. It doesn't replace a licensed attorney, doctor, or patient advocate.
Is it really free?
Yes - free during early access. You get the plain-English explanation, your deadline, the escalation steps, and a ready-to-send appeal letter at no cost.
Do you store my letter?
We don't store your letter on our servers, and it's never used to train AI. To analyze it, it's sent to our AI provider (Google Gemini) under an agreement that doesn't use your data for training. As with any online service, transient processing by our infrastructure providers may briefly occur.
Is my health information safe?
Yes. There are no accounts, no profiles, and no tracking pixels in your letter. The only thing we save is your email address - and only if you ask us to send you the appeal letter.
Will my appeal definitely succeed?
No tool can promise that. But a clear, on-time, well-argued appeal meaningfully improves your odds - and that's what we help you produce. Most appeals that get filed are overturned; most people never file.
What kinds of denials and plans does it handle?
Most US health-insurance denials - medical necessity, prior authorization, out-of-network, coding/administrative, and more - across PPO, HMO, Medicare, Medicaid, and ACA Marketplace plans.
How accurate is it?
Explanations come from a curated knowledge base, not the open web. Still, always verify the dates and details against your own letter before you act.
Do I need to create an account?
No. Paste your letter, get your answer. That's it.
What if the AI gets a detail wrong?
You can edit everything before you send it. Always double-check names, dates, and your deadline against your actual letter.
Who made this?
Denial Decoder was built for patients, by people frustrated that insurance letters are impossible to understand. Our goal is simple: help you push back.
