Why it happens
You have more than one plan, or the insurer has outdated info about your primary coverage.
What to do
Confirm which plan is primary, update coordination-of-benefits info with both insurers, and have the claim submitted to the correct payer first with the primary EOB attached.
Can you appeal it?
Yes - provide proof of which plan is primary.
